Next I accessed my resources by deciding what resources would be best, choosing a few primary and secondary sources. After, I processed the information, where I looked closely at the information from my selected sources and decided if they might be helpful towards my assigned topic. For example the main concepts like background information, certain effects from the topic, political views and perspectives. Then I organized the information in different categories, and developed my own ideas about them, which would help me form an outline for my research paper. Results: The Square Deal was President Theodore Roosevelt’s domestic program formed upon four basic ideas of conservation, regulating business monopolies, nforcing the anti-trust act, and supporting progressive ideas. A prompt response is one that follows the child's behavior as quickly as possible. Their actions are also very appropriate as they shape her interaction by matching the young child's behavior as closely as possible in terms of features like the loudness or softness of their voice, the level of emotion in their expression, and the kinds of gestures they make. 2.2 Health and Safety With regards to health and safety, the center is very much prepared and has accreditations and permits from different health agencies. These documents were shown to me. The measures they have installed include emergency policies, provisions, and education for staff, children, and parents. In managing injuries, for example, they follow these steps: survey the scene, assess the injured child, provide immediate care for the child including first aid, notify parents, obtain emergency medical care if needed and document and report the incident and actions taken. Issues with Cameras and Privacy in Modern Day Technology Scam Cams Nick Doran Introduction and Significance of Issue As a citizen, governmental laws have always played a major role in the way we conduct ourselves. Whether it’s when we vote or when we sit down in our car to travel, they always have a say or way to regulate us. When it comes to the motor vehicle, being a daily driver of suburban and urban areas are stressful enough. Within the last few years, the government has introduced a new X-factor causing many drivers anxiety and even possibly altering a driver’s ability to use their own intuition on being safe and respectful. This new x-factor can come in several forms of traffic violation cameras. The increasing number of cameras being put into effect raises concerns. Having a camera on every street corner leads to concerns about privacy and overreaching by the government in its ability to monitor the movements of people. Even worse, the accuracy and efficacy of these cameras can be called into question, meaning this invasion of privacy does not even have tangible benefits. Thes e cameras are truly an abuse of power by the government, and are an unethical used of technology in our society. A topic that people are very reluctant to realize is how quickly technology has advanced in the recent years. Of course the majority of our society is up to date with the latest version of the iPhone or the newest model of Dr. Dre Beats headphones, but not many people take a second and look around to realize that everything they do from the minute they wake up, to the minute they go to sleep is being monitored. Regardless if it’s the government monitoring them or a computer hacker sitting in his or her basement, your activity and whereabouts are being watched. With every installment of a traffic violation camera, another pair of virtual eyes is watching you. The most common cameras that are being used are Red Light Cameras and Speed Limit Enforcers. Currently, being used in smaller numbers are Bus Lane Cameras and Stop Sign Cameras. With so many flaws in this growing system of cameras, how can we possibly sit back and give away our right to privacy? True Purpose The true reasoning behind the outbreak of these cameras is pretty obvious, revenue. Both state and local government are defending these allegations by claiming that the cameras are for safety. According to the local government of Chicago â€Å"Red Light Camera Enforcement is designed to increase safety on Chicago streets. Cities across the country, and throughout the world, have been using the technology for many years.† (Red Light Camera Enforcement). While this defense seems nice on the surface, many studies have actually shown an increase in rear end collisions due to red light cameras. Drivers are becoming so indecisive and hesitant; the roads are becoming more dangerous. Many drivers are slamming on their breaks when the light turns from yellow to green out of fear of a camera flash following a fine, causing the car behind them to slam into the back of their car. It was found that in certain areas, the yellow light time was actually decreased, often below legal limit, in order to increase the amount of red light offenders. The increase in offenders clearly resulted in greater revenue to the government that issued the cameras. When the goal of increased safety is proven wrong and local governments such as Suffolk County, NY are â€Å"estimated $6.8 million a year† (Brown, J. 2014), it becomes evident that the dollar sign greatly out weights the cameras intended purpose of safety. In retrospect, an extra $6.8 million a year to the county may seem nice, a better plan should be put in place to raise these funds. This plan of traffic violation revenue seems more like a way of taking advantage of a poorly handled situation to get a quick fix of cash. Many individuals and organizations are fed up with these â€Å"scamras† (Werner, A 2012) and are leading successful oppositions of the use of cameras. â€Å"Brothers Michael and Paul Kubosh led a successful rebellion against red light cameras in Houston. The city took the cameras down more than a year ago. The brothers say the only reason cities install the cameras in is to make money. Its estimated that Houston earned $44 million during the four years the cameras were operating.† (Werner,A. 2012). There is no denying that the facts are in the numbers, these cameras are 100% appealing to anyone who rather focus on making money off others mistakes and than protecting us. Smile for the Camera Have you ever looked up on your way out of a grocery store and saw a flat screen television with multiple camera angles of you being displayed? Some may think well, the store has a right to know if someone’s stealing from them, while others may feel uneasy that they are being recorded in their pajama pants. Now think about all the other aspects of your life could have been learned from the time you woke up to the time you walked out of the grocery store by the government or any person or agency with hacking abilities. An outburst of use of Instagram, Twitter and Facebook on your smart phone early in the morning may suggest the time you woke up. The pictures you like and comment on suggest your interests. Software has the ability to turn your smartphone camera on and see through it from 100 miles away. Your neighbors’ security camera catches you walking out of your front door without locking it behind you. Now you get into your car, and every intersection from your home to the store has a camera on it. The cameras know: the route you choose to the store, the make, model, color, year and licenses plate of your car you’re in, if you’re in the car alone or with your kids, and if the camera takes high quality images, it may be able to recognize a wedding band on your finger that’s wrapped around your steering wheel determining if you are married or not. Don’t forget the security camera on the way out of the grocery store that recorded you forgetting to get milk. You may ask, what’s the big deal if the little old lady whole mails me my ticket gets to see pictures and a video of me in my car? The problem is anyone who wants those images badly enough, can get a hold of them. With most new red light camera systems, they are recording video 24/7, meaning all drivers (not just red light runners) are being recorded at all times. The video and still images are stored on privately owned computers servers without any verifiable audit trail of when the video is deleted. Meaning that it is unclear to what really happens to all this information. As a society, where do we draw the line to this invasion of our privacy by our governments? It’s Almost Always Accurate In today’s age we rely heavily on technology and it is safe to say, it doesn’t always work. Technology must be maintained, updated and sometimes restored. Sometimes it can even be defective from the start, â€Å"a setback occurred in 2003 when one vendor, who subsequently went out of business, was found to have provided equipment that was inaccurate. This led to negative publicity, suspension of camera use at three sites, and reimbursements of fines and demerit points to about 90,000 motorists.† (Delany, A. 2005) This is just one of many cases in which this system has failed right off the bat resulting in false accusations to a crime. Many of these cameras are recommended by manufactures, to have regular inspections by specialized technicians. A neglected system can result in similar outcomes as the one stated above. It seems that whatever department assigned to maintain these cameras, often does not. Resulting in thousands of issued tickets to many innocent citi zens that are invalid. Another case of this failed use of technology occurred in Chicago when their camera vendor, Redflex, was â€Å"fundamentally deficient, with little supervision such poor record-keeping that it is difficult to determine why the number of tickets issued spiked during period, as first reported by the Chicago Tribune.† Reflexes’ mishaps with â€Å"one detector at the intersection of Kimball, Lincoln and McCormick avenues was largely nonfunctional for years, meaning that the broken system â€Å"may have failed to have identify as many as 45,444 violations over a four-and-a-half-year period.†Ã¢â‚¬  With an uneducated society on this abuse of technology we quickly pull out are checkbooks to pay off these fines before a warrant is issued to the offender. We are forced to act out of fear rather than fighting for what is wrong and right. Psychology is Stronger then Law â€Å"Although many issues are raised in the numerous and constant debates on the revenue-raising aspects of speed enforcements, one key area of knowledge that can shed some light on it has yet to be highlighted, especially in the research literature. Do we really need to catch offenders to be effective? Can crashes be reduced simply by the presence of police enforcement without the need to issue tickets? Previous research has shown that even plywood replicas of police cars had an effect on driver behavior (Baker and Lawder, 1968) and simply giving a warning also had an effect on driver speed (Ennis, 1967). If the mere presence of police is sufficient to deter speeding, then there is no escape from the revenue-raising allegations† (Tay,R. 2010) The wrong approach is being taken to solve this safety problem. These cameras are not the answer. This plywood cop car study goes to show that the psychological presence of a police officer has a greater impact on how people drive. If a police officers presence if constantly know in a school zone, motorists will subconsciously become alert and be more aware of surroundings. A high majority of people throughout numerous studious have agreed that this would be the more effective method in accomplishing safer roadways for communities. Conclusion It is a hard point to miss that these Scamcams, are exactly that. Their intentions may seem great, but the way our government has chosen to implement them is just outright absurd. The notion that we are installing cameras on almost every major street corner is essentially giving another pair of eyes to whoever has the capability to use them. Whether it’s the national government monitoring, a college student with the self taught capability of hacking, or just your local government sticking you with a revenue boosting fine, you are being watched and there’s no ignoring that. We are allowing a problem to spiral out of control right in front of us. We are allowing our governments to encroach farther into our person space and privacy. Not too mention, they’re making a large sum of money off this program. Are their intentions still to make the streets a safer place or did the find a way to use this safety campaign as a cloak to scamming its people. Integration and Independence In the latter half of 1885 several European empires gathered together for a conference in Berlin. At this meeting of utmost importance, the various European powers laid claim to their African colonial territories, thus dividing the continent like a birthday cake among themselves. The new boundaries now united thousands of cultures, nations and ethnicities under the banner of white ethno-centricity. Thus began the final colonial dominion of Europe which would last well into the 20th century. Although the white minority has relinquished it?s stranglehold on Africa, the ramifications of hundreds of years of political dominion can be found throughout the continent. In 1962 during the heat of these independence movements, the world?s black population watched intently to see if indeed the African race could overthrow their colonial oppressors to bring about a new world order. Half a world away, men of African descent were fighting for an independence of their own. With the American Civil Rights movement in full swing, blacks everywhere stood on the brink of bettering the world for themselves and their children. The American essayist James Baldwin states in his essay ?Down at the Cross? the meanings of the words integration and independence. The Golden Rule ‘The Golden Rule and The Greatest Good’ what can they do? It was around 4 pm that Tosin Olumito and I got to the main campus of Ahmadu Bello University Samaru, Zaria after a long journey from Ilorin, Kwara State. Our mission was to process our admission having finished the Interim Joint Matriculation board Examination of which we obtained a very good result. In Ilorin, we had been directed to a friend who was a student in the school to put us through and to offer us accommodation for timeous processing and efficient follow up of the admission but alas! The school joined the 6 months strike called by ASUU. We knew they were on strike, the news we heard was that admission had started and that they would soon called it off to join some of their colleagues who had called their student to resume without a definite announcement by ASUU, which they eventually did after a week that we got to the university. We tried and searched every nook and cranny of the university to locate the friend we were directed to until somebody nimbled us that the stude nt we were looking for had traveled. All hope shattered, we looked at each other amazingly, we were now left to carry our cross and look for a panacea to this menace. We couldn’t go to a hotel neither do we have relatives within the vicinity. Tosin has a relative in Kaduna, which is 45 minutes drive from Zaria. I alluded that she should go to Kaduna to locate her relatives, that I would find a way of harboring myself. She wanted to disagree having thought of the risk, which I would be exposed to. She insisted on knowing where I would be so that she could check the following day to inform me whether I should come also to Kaduna or not . I told her that I would not be able to afford the transport implication which could adversely affect my pocket money because I was left with a token that could last for few days apart from my fare to Ilorin. She was convinced. As luck would have it, I met and told a Hausa student our plague who introduced us to his friend – Suleiman Nuhu Bala, a final year Economics student who was at the time writing his project. Understanding Mental Illness: Means for Lifting the Stigma As a victim of the debilitating mental illness clinical depression, I have a first hand knowledge of the terrible stigma attached to seeking medical help for this and similar problems. When the diagnosis was made, I told no one that I was seeing a psychologist. I feared what people would think of me and how they would react to one of their friends seeing a "shrink". Because mental illnesses are not well known and even less well understood, people tend to fear them. People who may be afflicted with mental illnesses often shy away from treatment because they fear that they will be labeled "crazy" or sent to an institution. I think that the stigma attached to mental health care could be easily lifted if regular psychiatric examinations were instituted, much the same as regular physicals. As people are exposed to the illnesses and begin to understand their origins and ways to help, this fear of people afflicted with the diseases should shrink (ha ha) and eventual ly disappear. As our country moves from a society made up of mainly manual laborers, to a society of white-collar workers, Americans find themselves with more and more leisure time. Often, if everything is going well in the world, society will look for problems to take the place of those that have been eliminated. For example, during the past ten years, Americans have had few really big problems, there have been no close to home wars, the economy is doing well, and unemployment has been dramatically down. Because of this, people have had large amounts of free time and energy, which was previously spent trying to work out larger problems and issues. For many people, this time is spent looking within themselves and often noticing things within their personality and psyche that would have been overlooked by earlier generations (onhealth.com/ conditions/cause). Statistics of today would lead one to believe that the occurrence of these illnesses has increased, however; awareness and the willingnes s of the victim to receive help has increased instead. There is less of a stigma attached to seeking mental health care than there has ever been before in this country. People are more willing to recognize and obtain help for their problems instead of ignoring them and going on with their lives as if nothing is wrong. With the continual exposure of the general public to these diseases, society is sure to place even a lesser degree of shame on the sufferer and his or her family.

Foundations of Nursing Practice Essay

A reflection on how the module content and associated practice experience has contributed to the student’s development as a nurse. This essay is a discussion on how the module content and practice experience has contributed to personal development as a nurse. Therefore it will focus on firstly the concept of individualised care and its relevance to nursing assessment and care delivery, secondly the ethical and professional issues that impact nursing such as confidentiality, consent and dignity. The essay will also focus on exploring the various communication models and the development of the therapeutic relationship between the nurse and the service user, and finally the organisation and delivery of care within the practice environment. With particular reference to a recent placement, at a palliative care nursing home, this essay will discuss Harriet, an 88 year old lady with chronic bronchitis and suffers with multiple sclerosis. For confidentiality purposes the names of service users have been changed to comply with the Nursing and Midwifery Council, Code of Conduct (NMC 2008). This assignment will firstly discuss the individualised approach to nursing, which developed in the USA during the 1950’s and 1960’s, which coincided with the development of the nursing theory and models of nursing which began to challenge the medical model of health care (Lloyd, Hancock, Campbell 2007). The nursing process can be carried out successfully by implementing a popular model used extensively in the UK originally by Roper et al Activities of Living model in which it is based loosely upon the 12 activities of daily living; however Henderson acknowledges 14 activities that people engage in (Kozier, Erb 2008). This model identifies any deficit in their care usually upon admission; it is reviewed as the care plans of the service user evolve; after which an intervention may be given to the service user. The nursing home that was attended for placement had person centred care plans in which they based them on the 12 activities of daily living from sleeping, eating and drinking to mobility and communication; these were short term care plans that were reviewed monthly. Another model that is frequently used described by Ellson (2008, pg22) is the Nursing Process, when it was initially identified; Yura and Walsh (1978) showed that it is a four stage cycle that begins with assessment. Harriet was assessed by the Doctor as the registered nurse in charge (RGN) noticed a change in her health; she had developed a wheeze when talking and had a chesty cough, the RGN suspected a chest infection. A care plan was then devised to follow up the assessment. Harriet was to be started on a course of antibiotics for a week, and was to be resumed on her nebuliser. The care was then implemented the following day during the morning drugs round. An evaluation of that care completes the cycle; in which Harriet was to be seen by the Doctor the following week after completing her course of antibiotics and was to remain on her nebuliser. Additionally, more than one stage can be occurring at the same time, for instance assessment may coincide with implementation (Carpenito-Moyet 2007). If goals are achieved after the first cycle, care maybe terminated or in some cases modified and the service user reassessed. (Ellson 2008, pg22) This process is designed to enhance systematic care, drive communication amongst team members and encourage continuity (Mason 1999 cited in Habermann, Uys 2006).The RGN wrote in Harriet’s daily report and updated her care plans, identifying her change in medication and change in her health. Throughout nursing there are many ethical and professional issues that impact on it from confidentiality, consent to respect and dignity, to clear and accurate records and working as part of a team. The Nursing and Midwifery Council Code of Conduct (NMC 2008) clearly states that nurses should ensure they gain consent (NMC 2008) from the service user this can be anything from examining, providing care and giving treatment. Consequently a nurse has to be prepared if a service user declines their help as they have a right to refuse treatment based on knowledge of the outcomes and risks (Wilkins and Williams 2008). Informed consent is a communication process between the provider and the service user; this is now recognised as a professional standard of conduct (Westrick and Dempski 2009). However if there is a case where the service user does not have the mental capacity to give consent, they are protected by the Mental Capacity Act 2005 in which during the decision making process their rights and interests are accounted for (Griffith and Tengnah 2010). My first task at placement was to try and feed Harriet, I introduced myself alongside a health care assistant and asked if I was able to feed her lunch, I was refused, on the grounds that I was a fresh face and that she did not know me so I let the health care assistant feed her. The following day after assisting the RGN with Harriet’s medication, she kindly said to me that she would let me feed her today if I was able to; so at lunch time I went along with another health care assistant to feed her. Another important ethical issue is maintaining a person’s dignity as Watson (1994) citing (Watson 2008) states that maintaining human dignity is a vital nursing duty and function, that needs to be recognised and respected in which people make to their own care and well being (NMC Code 2008). This can be associated with many aspects of care such as bed bathing, stoma care, peg tube care as Westrick and Dempski (2009) go on to say that it is down to the service user to say what is to be done with his or her body. Fenton and Mitchell (2002) cited in Franklin, Ternestedt and Nordenfelt (2006) argue that elderly people receiving care regarding dignity is a state of physical, emotional comfort, subsequently when this is not always adhered to it can leave the service user feeling embarrassment, shame, humiliation, foolishness and degradation (Mairis 1994 cited in Watson 2008). However nurses themselves can become emotional if the standard of care given is not sufficient this is supported by the Royal College of Nursing survey (2008) asking nurses for their views on dignity in care in which over 80% said they sometimes or always left work distressed due to not being able to deliver the quality of care they thought they should give (Gallagher, Tschudin 2010). When asked to give a bed bath to Harriet, I made sure that only the area being cleaned was exposed thus covering up other private areas in case anybody was to enter her room. When Harriet was seen by the Doctor regarding her chest infection I treated the information sensitively and maintained confidentiality with all her health records. The presence of the therapeutic relationship lies at the heart of patient centred nursing. However the nature of this one to one relationship is very dependent on the context in which nursing care is delivered (McCormack 2004 citing O’Connell 2008). Therefore the building blocks for this relationship should focus on genuineness, empathy and respect, which should leave the service user feeling supported as well as listened to, whilst the nurse feels value in their role (Dossey, Keegan, American Holistic Nurses Association 2008) Whereas Bynum-Grant and Travis–Dinkins (2010) go on to say that whilst the therapeutic relationship is at the core of nursing it is the knowledge and skill along with the caring attitudes and behaviours applied that build the foundations of this relationship. I spent a lot of time with Harriet getting to know her, and building a relationship of trust with her, in order for me to attend her personal care needs, give her medication orally under dire ct supervision. This assignment will now discuss the effective communication skills throughout nursing that helps practitioners to engage with the service user, by making sure arrangements are met for people’s language and communication needs (NMC Code 2008) Studies have shown that the relevant communication means given to an individual can improve well-being (Bell 1996, Happ 2001 cited in Batty 2009). Means of communication that have proved successful are non verbal techniques such as writing, drawing or by gesture, therefore communication aids have been made available such as providing a pen and paper, or alphabet charts (Batty 2009). Effective communication is seen as a fundamental competence required for registration as a nurse (Nursing and Midwifery Council 2004 cited in Timmins 2009) Timmins and Astin (2009) also goes onto explain that continuity of care supports high quality communication as it builds up the relationship between nurse and service user and is a main feature of patient centred care. Crouch and Meurier (2005) cited in Cox and Hill (2010) defines communication as a two way process in which information is transmitted and received. However listening to service users and their families is central to the communication process (Timmins and Astin 2009) Harriet could communicate easily, her hearing was slightly impaired therefore you had to speak loudly and clearly; she was on eye drops as her sight was deteriorating and she was very well spoken and if I was to say something incorrectly she would be very quick to correct me. Harriet however was slightly confused and sometimes repeated things she had said a few moments ago, she also had imaginary friends she would refer to whilst talking to me. This essay will now explore the delivery of care in nursing which is very important as this can impact the service users’ experience. There are three ways in which care can be delivered; this is by primary care, task allocation and team nursing. Primary nursing began in the 1970’s as a way to overcome dissatisfaction with functional and team nursing’s emphasis on tasks that directed nurses’ attention away from holistic care of the client (Huber 2006). Walsh and Crumbie (2007) explain that primary care nursing involves one nurse being liable 24 hours a day for all care delivered to a patient. However Thomas (2006) identifies this is not always necessary as care can be delegated to other nurses or health care assistants †¦ yet the primary nurse carries responsibility for writing care plans and ensuring that long term goals are met. Skelton (2001) cited in Timmins and Astin (2009) suggest that this approach fosters autonomy and gets them involved with their care rather than health professionals dictating their care to them. Within the nursing home the primary nursing approach to care was not suitable as there were too many service users for one nurse to focus all their attention to at one time. Many health care settings would deny using task orientated care however in reality it still goes on and tends to induce apathy and reduce team morale. Task allocation on the other hand may be the most suitable way of allocating the workload (Thomas 2006) especially during staff shortages or certain health care settings. Despite the nursing home having person centred care plans and making individualising the care needs, the nursing home still took on the task allocated approach as all the residents had breakfast by 8am and were washed and dressed by 9am. However as Harriet was unable to feed and dress herself due to having multiple sclerosis she was often left until last to have these needs addressed as two health care assistant’s were required. Team nursing was developed in the early 1950’s it was designed so that staff strengths can be used to the maximum and aids group productivity and growth of team members. By using this system nurses should still be able to provide individualised nursing care (Lloyd, Hancock and Campbell 2007) therefore nurses take on certain roles such as temperature, medication or a nurse for the right side of the ward (Kalisch and Kalisch 1978; Reverby 1987 cited in Huber 2006). Communication is therefore key for this model to be effective as the team leader continuously evaluates and communicates changes of the patient to the team members (Zerwekh, Claborn 2006). It also allows the nurse to delegate patients to the strengths of the staff with what their care focuses on (Tiedeman and Lookinland 2004 cited in Zerwekh, Claborn 2006). Unfortunately the care given can become fragmented and thus ineffective and productivity decreased among team members if there are staff shortages. Harriet’s care followed this approach, as she was bed bound the arjo hoist was the only way of transferring her from bed to chair; this required two people, as did log rolling Harriet for her bed bath so she was cleaned effectively and appropriately. I feel the module content has contributed to my development as a nurse as it taught all the relevant information needed for my first placement. I was able to see how the theory coincided with the practice which also contributed to my development as a nurse. For example I was able to see how the individualised care approach was introduced into the person centred care plans used within the nursing home. I was also able to see how the therapeutic relationship was built amongst the nurses in charge, the health care assistants and other members of the multidisciplinary team, they each had their own style of approaching the service user in regards to their attitudes, body language and behaviour, this I was able to pick up and use myself which made me handle situations better. The module content went into great depth and was given in a variety of learning styles which suited my learning pattern having come straight from school; I feel the type of learning given has strengthened my ability to learn and the relevant theory made me feel more confident when attending placement. Having learnt the theory I was able to put some of this knowledgeable experience into practice, as having had no previous experience within health and social care I felt uneasy at first but as more faith and confidence was put into me via my mentor and other staff members, this helped me feel more involved and more comfortable with working within this environment giving me the confidence boost I needed. Throughout the placement I was guided throughout every task that I had to carry out, I was taught it first by mentor and then throughout the weeks I had to improve on what I had learnt, the registered nurses I shadowed were very supportive and approachable which helped me greatly. In conclusion this essay has discussed the concept of individualised care by incorporating the nursing process and models that evaluate the service user’s health and identifies the care needed and any deficits in their care. Nurses uphold a reputation in which the ethical and professional issues are the foundations of this. As explored throughout this essay it shows that confidentiality, consent and dignity are fundamental throughout nursing and need to be incorporated into the care provided by the nurses. Various models of communication have been identified throughout this essay its relevance shows how the nurse must interact with service user, this is very important as the service user needs to be able to express their concerns of their health either verbally or non verbally. The therapeutic relationship is at the epicentre of nursing as this can impact a service users experience within a health and social care setting. The essay has also explored the organisation and delivery of care needed to be most effective throughout nursing. Overall the essay has focused on various fundamental aspects of nursing that contribute to the way care is delivered to service users. Reference List * Batty S.
